Are you or someone you know & love aging or having a medical decline that can change the future?

Have you or a loved one gone through a recent medical crisis that has led you to want and not want certain medical treatments?

Then let’s talk about it! and create an Advanced Care Plan or ACP

Advanced Care Planning is a person-centered medical plan that ensures that if a person becomes too ill or unable to speak for themselves, their medical team, support system, and designated health care agent will know their medical wishes and act accordingly. ACP also discusses types of treatments, end-of-life wishes, and arrangements. It brings about understanding and education about illness, clarity, and peace of mind.

In my years of experience, I’ve found that family members, caregivers, nurses, therapists, and even doctors have a hard time telling a person that their time is limited.

Understandably.

I have also learned that being proactive and having open communication with your medical and support system leads to less grief and inter/ra-personal conflict.

Acknowledging our existence and death is, for most, an uneasy & unfamiliar topic.

With years in the medical field working with individuals who are aging or terminally ill and becoming certified through Griffin Hospital as an Advanced Care Planning Facilitator, let Rebecca assist you in turning an uncomfortable conversation into an empowered, clear, and humbling experience.

This service is separate from psychotherapy as it is specific to advanced care planning wishes. The patient’s support system or a person to designate wishes should be present to discuss the patient’s wants, needs, and boundaries.

ACP documents will be provided to the patient.

The practice bases rates on a sliding scale as well as intricacy on needs. Rates would be discussed with the payer prior to services given.

Resource Planning

Firstly, what is Resource Planning?

Resource Planning services are personalized referrals for short—and/ or long-range services after a discussion with the patient on their strengths, wants, functionality, financial eligibility, status, support system, and needs.

This is a guaranteed two-visit service.

As insurance does not cover resource planning, this is a Private-Pay-Only service. The fee for a 60 to 90-minute two-visit bundle is $200. Revisits are $250 with personalized referral. The clinician should provide a subjective via Resource Planning Service of Consultation pursuit.

Rebecca Sember is not an eldercare lawyer.

If Rebecca Sember believes an eldercare lawyer referral is needed due to the intricacy, patients’ best interest, an elder at risk, or other situational needs, she will provide a referral, and further continuation of services will be discussed.

This service does not cover completing or applying for services. If you’re interested in that, please see the Authorized Representative section.

Clinical LCSW Supervision

Rebecca offers clinical supervision to social work practitioners working to obtain their clinical license, LCSW.

As per Connecticut State, a social worker must have 100 clinical supervision hours with an LCSW prior to applying and taking the exam. If you do not have a social worker who can provide hours or if your agency does not allow individual supervision, this service would be perfect for you.

This is for social workers only. Private Pay only. There are options for a lump sum flat fee or recurring billing.
